Glass: Sits on the very top. Does nothing except cover the layers below...and of course it frequently cracks.
Digitizer: Senses your finger touches. On the iPhone 5 and above it's built into the LCD. For the iPhone 4 & 4S it's a separate part glued directly to the glass. When broken, all or part of the screen may not respond.
LCD/OLED: Displays the picture. For the iPhone 5 and above, it also has the digitizer built into it. When broken, your phone's screen won't display properly. It might be black, white, rainbowed, or a combination of these.
